Columbia Post Acute creates an individualized care plan for each of our patients. We tailor each of our services to create a comprehensive care plan concentrating on improving the quality of our service that reduces medical complications as well as minimizes the need for aggressive medical support and resources.

Wound Care

Columbia Post Acute offers an exceptional wound care program with an integrated system of professional, clinical, and technical resources designed to treat a variety of wound conditions. We provide care for diabetic arterial/venous ulcers and pressure ulcers. We also cover wound management, IV therapy, post surgical care and wound vacs.

Post-Acute Rehabilitation

Our patients can have a variety of specific rehabilitation needs. Our physical, occupational and speech therapists provide personalized treatment to our patients to improve their level of functioning. Our therapists motivate and inspire our patients to attain their goals and to reach their level of independence in order to assure a safe discharge.

Social Services

Our Social Workers provide essential information to our patients and family members. They partner with our care coordinators and therapists to assist with care and discharge planning for our patients. Open communication is maintained between the facility, families and patients. They also act as a resident advocate, making sure that the patient’s and family’s needs are met.

Activities Program

Columbia Post Acute’s Life Enrichment Coordinator’s goal is to make each of our patients have an enjoyable stay with us. She creates social programs that are tailored to our patient’s needs. A colorful and easy to read activity calendar is handed out daily as well as prominently displayed for all patient and their families to see. Activities include Bingo, arts and crafts,  live entertainment, Popcorn and movies, ice cream socials, games and more.
